If you are a semiweekly depositor and do not include Form 941 Schedule B, the IRS may treat your Form 941 as incomplete and issue a notice.  To attach schedule B, you may need to submit an amended Form 941 (Form 941-X) to correct the issue or provide the missing Schedule B information. 

Fix Missing Schedule B: 
  • Use Form 941-X to attach the missing Schedule B. 
  • The total liability on the corrected Schedule B must equal the total tax reported for the quarter. 
  • If you do not file 941-X, the IRS will likely send a letter requesting the form, as they will have received your tax payment but not the breakdown of liability.
